Job Description:
The service receives referrals from GP and other wider agencies. Assessments are carried out by the Mental Health Practitioners (MHPs) with involvement from the Consultant if required for specific clinical reasons. Cases taken on by the team are then co-ordinated by the MHP's with input from other members of the team including the Consultant as required.
